Welcome to New Beginnings Ob/Gyn, PC. Dr. Brenda J. Hines and staff would like to thank you for choosing our practice to take care of your gynecological and obstetrical needs. To make sure your appointments go as smoothly as possible, please carefully read the information below.
We are accepting new patients. Annual wellness exams for new patients will be with one of our physician assistants, and availability is about two months out. New patients with a specific problem can usually be seen in three to four weeks.
Please have your photo ID and insurance card ready at EVERY appointment. Also find out which lab your insurance requires you to use. Otherwise you will get the bill for any lab tests. Per your insurance carrier, we require the collection of co-payments at the time of service. We accept checks, cash, and major credit cards.
Please assist us by being on time for your appointment. A delay in your arrival could result in the need to reschedule your appointment. If for any reason, you are unable to keep your appointment, please call the office at (540) 370-4380 at least 24 hours in advance. Our office enforces a $50.00 fee for any appointments not cancelled or rescheduled prior to your appointment time.
Per your insurance company, we are required to collect co-payments, co-insurance and deductible amounts. If you have difficulty paying in full, please contact our billing office at (540) 370-4380 to discuss making payment arrangements. If a personal check is returned for non-sufficient funds, you will be responsible for a $35.00 returned check fee as well as the original amount. This must be paid in full by cash, money order or credit card.

If you would like to request a refill on your current prescription, you must contact your pharmacy and ask that they ePrescribe the refill request to our office. We are no longer accepting verbal requests from the patient or requests sent by fax from the pharmacy. Please note that any prescription refill is subject to the provider’s approval.
Information and Insurance Changes
It is important that you maintain updated records in our office. If any of your personal information such as address, phone number(s) or insurance plan changes, please contact us. If your insurance plan changes, please check to ensure that we are currently accepting your new insurance. Remember that your insurance coverage is a contract between you and your insurance company. You are ultimately responsible for payment of your account and knowledge of what your insurance policy covers.
Your medical record is strictly private. We do not reveal information regarding your health to your employer, friends or relatives without your written permission. We only release records with your signed consent or where required by law. Your signed consent is also required if you want a personal copy of your records. We are compliant with the HIPAA (Health Insurance Portability & Accountability Act) privacy rules and have a published “Notice of Privacy Practices” available in our office. *There will be a $15 fee for the completion of FMLA and short-term disability paperwork.
